Abstract Background Isolated common peroneal neuropathy (CPN) is the most lower extremity mononeuropathy and third overall in adults. It often associated with multiple aetiologies, such as trauma, hereditary neuropathies iatrogenic causes. We report a rare case of acute bilateral foot drop late adolescent female following rapid weight loss attributable to intermittent fasting diet. Case presentation A 19-year-old presented worsening intentional by She lost 20% her baseline bodyweight within period 4 months. Systemic disease metabolic screenings for differentials were unremarkable. Electrophysiological studies revealed severe entrapment involving nerves (CPN). Her confounding risk factors CPN injury include contact sport activities habitual leg crossing during sitting. Following dietary modification physical therapy she achieved complete recovery weakness function on follow-up. Conclusions In patient PN loss, prognosis good neurologic can be attained through combination cessation extreme diet therapy.
